Square D GFCI. This is the Qwik-Gard GFRP115 ground fault circuit interrupter. It is mounted in an enclosure and comes with a 10' 12-3 SO cord. This works and has been tested. I don't think that the instructions mounted inside the cover have anything to do with the current setup of the box enclosure so disregard them.
